Client Waiver of Liability & Informed Consent
1. Acknowledgment of Services
I understand that services provided by Honey Bee Collective LLC may include Reiki, intuitive or spiritual guidance, coaching, meditation, Neuro-Linguistic Programming (NLP), breathwork, and other holistic or energy-based practices. I recognize that these offerings are intended to support personal growth, self-awareness, relaxation, and energetic well-being, and are not substitutes for medical or psychological diagnosis, treatment, or therapy.
2. Nature of Services
I understand that the practitioner is not a medical doctor, psychiatrist, psychologist, or licensed therapist, and that no medical claims are made. These services are complementary to traditional care, not a replacement for professional healthcare. I have been advised to consult with a qualified healthcare provider for any medical or mental health concerns.
3. Voluntary Participation
I understand that my participation in any session or program through Honey Bee Collective LLC is voluntary, and I may choose to discontinue at any time. I acknowledge that all results are unique to the individual and depend on my own openness, commitment, and personal growth process.
4. Breathwork & Somatic Practices
I understand that breathwork may involve deep breathing, emotional release, or physical sensations. I affirm that I am in good physical health and have disclosed any relevant medical conditions (such as cardiovascular issues, asthma, pregnancy, or seizure disorders). I take full responsibility for monitoring my comfort and safety throughout all practices.
5. NLP & Mindset Coaching
I understand that Neuro-Linguistic Programming (NLP) and mindset coaching are tools for reframing thought patterns and personal development, and do not constitute psychological counseling. I understand that I am responsible for my own interpretations, insights, and actions.
6. Confidentiality
All sessions are confidential. Information shared will not be disclosed without my written consent, except as required by law or to protect against immediate harm.
7. Liability Release
I hereby release, indemnify, and hold harmless Honey Bee Collective LLC, its owner, and affiliates from any and all liability, claims, demands, damages, or causes of action that may arise from my participation in any session, class, or program. I understand that I assume full responsibility for my own health, safety, and outcomes.
8. Consent
I confirm that I have read and fully understand this waiver and informed consent. I voluntarily agree to participate in the services provided by Honey Bee Collective LLC and accept full responsibility for my participation and results.
