Client Waiver of Liability & Informed Consent
By booking a session with Honey Bee Collective LLC, I acknowledge and agree to the following:
Nature of Services
Honey Bee Collective offers complementary wellness services, including Reiki, Reiki-infused Breathwork, intuitive guidance, meditation, Neuro-Linguistic Programming (NLP), coaching, and other holistic practices. These services are intended to support relaxation, personal growth, self-awareness, and overall well-being. They are not intended to diagnose, treat, cure, or prevent any medical or mental health condition and are not a substitute for professional medical or psychological care.
Breathwork Contraindications
I understand that Breathwork is a physically and emotionally engaging practice and may not be appropriate for everyone.
I confirm that I do not currently have any of the following conditions unless I have discussed them with Honey Bee Collective and obtained written clearance from my healthcare provider:
- Pregnancy
- Significant cardiovascular disease
- Uncontrolled high blood pressure
- Epilepsy or seizure disorders
- Glaucoma or detached retina
- Recent major surgery or serious injury
- Severe respiratory illness that would make Breathwork unsafe
- A history of psychosis or other medical or mental health conditions that could make participation unsafe
If I have questions about whether Breathwork is appropriate for me, I understand it is my responsibility to consult my healthcare provider before participating.
Personal Responsibility
I acknowledge that participation is voluntary. I accept full responsibility for my own physical, emotional, and mental well-being before, during, and after any session. I understand that I may pause or discontinue participation at any time.
Assumption of Risk
I understand that holistic wellness services, including Breathwork, may involve physical sensations, emotional release, or temporary discomfort. I voluntarily assume all risks associated with participating.
Release of Liability
To the fullest extent permitted by law, I release and hold harmless Honey Bee Collective LLC, its owner, employees, contractors, and affiliates from liability for any injury, loss, or damages arising from my voluntary participation in these services, except where prohibited by law.
Acknowledgment
By submitting my booking, I acknowledge that I have read and understand this Client Waiver of Liability & Informed Consent. I confirm that I am voluntarily choosing to participate, that I meet the health requirements for the services I am booking, and that I accept full responsibility for my participation.
